NHS England's 2025/26 QOF reforms prioritised cardiovascular disease prevention with funding redirected to CVD indicators and enhanced point allocations. The dashboard described below covered 11 indicators across cholesterol management, blood pressure control, antiplatelet therapy and smoking cessation. Indicator codes, weightings and business rules may change under the 2026/27 framework.

Statin prescribing and cholesterol target achievement for patients with established CVD (CHD, PAD, Stroke/TIA) or CKD.

Secondary prevention BP control for patients with history of stroke or transient ischaemic attack.
Blood pressure management for diabetic patients as part of comprehensive cardiovascular risk reduction strategy.
Systematic smoking status recording for patients with long-term cardiovascular and other conditions within the preceding 12 months.
